Non-urgent advice: Statement of Purpose

This is a Statement of Purpose for the Chapelgreen Practice which sets out the following information:

  • The full name of the service provider and of any registered manager together with their business address, telephone number, and where available electronic mail addresses
  • The legal status of the service provider
  • Details of the locations at which the services provided for the purposes of the regulated activity carried on.        
  • Our aims and objectives in carrying on the regulated activity.
  • The kinds of services provided for the purpose of carrying on of the regulated activity.
  • The range of service users’ needs which those services are intended to meet.

Chapelgreen Practice is a General Practice Partnership open to all patients living within our Practice boundary in Chapeltown, High Green and the surrounding areas and to patients outside our practice boundary meeting certain conditions. We work in partnership with our patients to provide medical care for our patients.

We operate over 2 sites; the main site is Burncross Surgery, and our branch site is High Green Health Centre.  The practice is part of Network North PCN and we use Sheffield GP Collaborative for our out of hours service, who use the same clinical system. This allows continuity of care therefore always providing the best possible care.

We are a Personal Medical Services (PMS) Practice offering Primary care services for the diagnosis and prevention of disease. We help patients to manage their health and prevent illness. Our GPs, nurses, and wider clinician group, assess, diagnose, treat, and manage illness.

Our services include screening for some diseases and promotion of general health and wellbeing. Our health professionals strive to support and represent patient’s best interests, and ensure they receive the most appropriate health and/or social care. Our health professionals provide referrals and advice to direct patients, where appropriate to other health services, and work closely with other healthcare professionals. They arrange hospital referrals, admissions, and referrals to other services, including specialists within the community, who provide patient care. This may involve sharing of information where needed. We also collect and record important information from other healthcare professionals involved in the treatment of our patients.

Our GPs are also involved in the education and training of doctors, practice staff and other healthcare professionals.

Both locations offer services to the whole practice population. Both premises is a purpose-built facility with access for disabled persons providing additional medical services. Each site has all the equipment required to meet its contractual obligations.

The Practice must provide essential services for people who are sick or perceive themselves to be sick with conditions from which recovery is expected, chronic disease management and general management of terminally ill patients.
